Total Knee Replacement

Total knee replacement is a surgical procedure to cure the problems of knee joints caused by osteoarthritis as well as rheumatoid arthritis. As knee joints are the most complex joint of the human anatomy, it suffers from various acute injuries and diseases. The problems of knee joints like meniscus tears, osteoarthritis, cartilage defects, and ligament tears are cured by total knee replacement.

Simply total knee replacement means the replacemnt of the diseased or damaged joint surfaces of the knee with metal and plastic components to give normal and healthy movement to the knee. Those have osteoarthritis of the knee suffer from some pain, stiffness, or swelling in the knee joint on most days. If general treatment does not relieve pain and improve knee function, total knee replacement surgery is recommended. In case of female, osteoarthritis of knee joints occurs in the age group 40 to 60. The male age group that requires replacement of knee is 45 to 65 and above 65.

The procedures for total knee replacement have been modified in last few years. Under epidural, spinal, or general anesthesia, the total knee replacement is performed by the doctors. The timings of all total knee replacement are not alike, the operative time differs. The typical knee replacement time is 80 minutes.

In total knee replacement surgery, the front of the knee is exposed with detachment of the quadriceps muscle from the patella. The collateral ligaments are preserved; the cartilages and anterior cruciate ligament are removed. Metal alloys are cemented into the bone or fixed by using polymethylmethacrylate cement. The other processes include a round ended implant on femur, insertion of high density polyethylene surface, balance of ligaments etc.

The implant manufacturers are different for total knee replacement surgery and different instrumentation and technique are required for each replacement. The common variations are cemented and non-cemented components. In some cases post primary surgery is recommended to the patients undergoing knee replacement surgery. It is generally revised after 15 to 20 years. In this surgery, computer assisted surgery procedure is also used.

Knee Arthroplasty is one of the major surgeries done in total knee replacemnt. Pre-operative tests like complete blood count, electrolytes, APTT and PT to measure blood clotting, chest X-rays, ECG, and blood cross-matching for possible transfusion are done.

The post-opretavie rehabilitation inclue continuous passive motion, the Post operative hospitalization, etc. Usually the patient need three months to recover from it completely and sometimes it may take more time.
